Telehealth Informed Consent Policy

This Telehealth Consent Policy describes how telemedicine visits with our practice are conducted, the benefits and risks involved, and the rights and responsibilities of our patients. Please review it carefully.

Telehealth Informed Consent

Effective date: August 30, 2026

What I am consenting to

I am choosing to receive medical care from Billie Jean Health through telemedicine. My care is provided by Lauren Leavitt, MD, or another licensed clinician of the practice.

Telehealth services may include asynchronous care, such as submitting medical history, health questionnaires, and other health information for clinical review, as well as synchronous care, including telephone or live video consultations with a licensed healthcare provider when clinically appropriate.

I understand that a telemedicine visit takes place by video, audio, and secure messaging rather than an in-person examination, and that it is not the same as being seen in person. My consent is voluntary. I may withdraw it at any time, and doing so will not affect my right to seek care in the future.

Benefits and risks

Telemedicine has potential benefits, including easier access to care, lower cost, shorter waits, and the ability to be seen from home.

It also has potential risks. The connection may be interrupted or the video or audio quality may be poor. Information transmitted during a visit may not be sufficient for my provider to make an appropriate medical decision, which may lead to a delay in evaluation or a recommendation that I be seen in person. Technical failures may delay or prevent care. Despite reasonable safeguards, no electronic transmission is completely secure, and unauthorized access to my health information is possible.

I understand that either my provider or I may end a telemedicine visit if the connection is not adequate for the situation.

Limits of a telemedicine examination

I understand that telemedicine has limitations with respect to physical examination. The examination portion of my care is limited to what can be observed by video. Some elements of a physical exam — physical tests, examination of certain body parts, and vital signs — may be performed at my location by someone else at my provider's direction, may be self-reported by me, or may not be performed at all.

I understand that my provider may determine that telemedicine is not appropriate for my situation and may recommend or require that I be evaluated in person before care can continue.

Emergencies and urgent care

Billie Jean Health does not provide emergency care. Telemedicine services offered through the practice are not an emergency service and are not monitored continuously.

If I am experiencing a medical emergency or an urgent medical problem, I will call 911 or go to the nearest emergency department. I will not use messaging, email, or the patient portal to report an emergency.

I understand that messages sent to the practice are answered during business hours and that I should not expect an immediate response. Patients can expect a response within 24 hours.

Where I must be located

Telehealth services are currently available only in jurisdictions where the treating healthcare provider is appropriately licensed to practice medicine and where the applicable pharmacy is authorized to dispense and ship prescribed medications.

During registration, patients may be asked to provide their residential address and ZIP code. This information is used to determine service availability, treatment eligibility, provider licensure, and pharmacy availability.

Because provider licensure, pharmacy licensing requirements, prescribing regulations, and state laws vary, the availability of providers, medications, pharmacy services, and treatment options may differ based on a patient's location.

If telehealth services or prescription fulfillment are unavailable in a patient's jurisdiction, the patient should be notified before clinical services are provided.

I agree to accurately disclose my physical location at the start of each visit. I understand that the practice may be unable to provide care, or may need to reschedule, if I am located in a state where my provider is not licensed.

Medications and prescriptions

I understand that paying for a visit or a membership does not guarantee that I will be prescribed any particular medication, or any medication at all. Whether to prescribe is a clinical decision made by my provider based on my individual evaluation.

I understand that some medications prescribed through the practice may be compounded. Compounded medications are not FDA-approved. The FDA does not review compounded drugs for safety, effectiveness, or quality.

I understand that certain medications, including controlled substances, are subject to additional federal and state requirements and may require additional steps or an in-person evaluation before they can be prescribed.

I understand that no specific outcome or result has been promised to me.

Laboratory testing

I understand that my care may require laboratory testing before treatment begins and at intervals during treatment, and that my provider may decline to prescribe or continue treatment if required testing is not completed.

Other people present during a visit

If another person is present with my provider during a visit, I will be told who they are and why they are present. I agree to disclose whether another person is present with me.

Anyone present during a visit is expected to keep what is discussed confidential. Because I will be informed of anyone else's presence, I have the right to:

  1. omit details of my medical history or examination that are personally sensitive to me;
  2. ask non-medical personnel to leave the visit; and
  3. end the visit at any time.

Recording

Visits are not recorded by the practice. I will not record a visit without my provider's knowledge and consent.

Privacy

My health information is handled as described in the practice's Notice of Privacy Practices. I understand that the risks described above include the possibility of unauthorized access to information transmitted during a visit.

To protect my privacy, I will not share my login information or video visit links with anyone not authorized to attend my appointment.

Continuity of care

I understand that Billie Jean Health provides focused care in specific areas and is not a substitute for a primary care physician. I am encouraged to maintain a relationship with a primary care provider, and I authorize the practice to share information with my other providers as described in the Notice of Privacy Practices when doing so supports my care.

Alternatives

I understand that I may choose to receive care in person from another provider instead of by telemedicine.

Acknowledgment

I have read this consent, I have had the opportunity to ask questions, and my questions have been answered to my satisfaction. I voluntarily consent to receive care from Billie Jean Health by telemedicine.
